Instructions for Online Registrations
1. Online registration for new students will commence on Sunday, June 7th and end on Sunday, June 21st.
a. If you need assistance registering online, please bring your phone, iPad or laptop along with the documents listed below to the Cultural Center on Sunday, June 7th between the hours of 9 am and 5 pm. We will show you how to fill out the form.
2. Before you begin the registration process, please ensure the student meets the following requirements.
a. The student is in 9th – 12th grade.
b. The student can read, write, and understand English fluently.
c. The student is baptized in the Roman Catholic Church.
3. Please have the following readily available while filling out this form.
a. Have your child with you when filling out the registration form.
b. Have your Sunday envelope number available.
c. If your child has special needs, please have a picture of the student’s most current IEP documentation available on your phone/computer.
d. Year 1 Have a clear picture of the student’s birth, baptism, and first communion certificate on your phone/computer.
e. Year 2: Have a debit or credit card available to make your $350 payment in full after submitting your form.
f. Year 2 Transfer Students: If your child is applying to enter Year 2 at Christ Cathedral from another parish, please have a photo of the student’s transfer letter on your phone/computer. The transfer letter must be signed by the Pastor, and the Director of Religious Education or Confirmation Coordinator of the parish where the student completed his or her first year of formation. Transfer Students must email their Letter of Transfer to [email protected]
4. Please fill out a separate registration form for EACH child.
5. Please pay close attention to the Confirmation Program Requirements for Year 1 and Year 2 outlined in the form. If you believe that you will not be able to comply with the program requirements, please do not submit your registration form.
6. We would also like to remind you that priority will be given to ACTIVE registered parishioners of Christ Cathedral Parish. If space remains after the registration deadline of Sunday, June 21st, non-parishioners will be considered in the order received.
7. The Confirmation team will begin reviewing registration forms the week of August 11th and will send an email to applicants informing them whether their registration has been approved or denied. Remember, space is limited and we are required to give priority to those who are ACTIVE registered parishioners.
8. ATTENTION: Your registration form will be DENIED if you DID NOT provide the following:
a. The student’s Birth Certificate.
b. The student’s Baptism Certificate.
c. The student’s First Communion Certificate. (if applicable)
d. A copy of the student’s most current IEP documentation. (if applicable)
e. If you did not make the $350 payment in full within 5 business days of receiving the payment link.
f. For those wishing to transfer from another parish, a copy of the student’s year 2 transfer letter signed by the Pastor AND the Director of Religious Education or Confirmation Coordinator from the parish where the student completed their year 1 formation. (if applicable)
9. ATTENTION: NO REFUNDS will be given after the first day of class, September 20th.

Instrucciones para la Inscripción en Línea
1. Las inscripciones comenzarán en línea el 7 de Junio y terminará el 21 de Junio.
a. Si necesita ayuda para registrarse en línea, traiga su teléfono, ipad o computadora portátil junto con los documentos que se enumeran a continuación al Cultural Center el 7 de junio entre las 9 am y las 5 pm. Le enseñaremos cómo completar la forma de registración.
2. Antes de comenzar el proceso de registración, asegúrese de que el estudiante cumpla con los siguientes requisitos.
a. El estudiante está en el grado 9 al 12.
b. El estudiante puede leer, escribir y comprender inglés con fluidez.

e. Año 2: Tenga una tarjeta de débito o crédito disponible para realizar su pago completo de $350 después de enviar su formulario.
f. Año 2: Si su hijo solicita ingresar al Año 2 en la Catedral de Cristo de otra parroquia, tenga una foto de la carta de transferencia del estudiante en su teléfono/computadora. La carta de transferencia debe ser firmada por el Párroco o Director de Educación Religiosa o Coordinador de Confirmación de la parroquia donde el estudiante completó su formación el primer año.
4. Complete UNA forma de registración por separado para CADA niño.
5. Preste mucha atención a los requisitos del programa de confirmación descritos en el formulario. Si cree que no podrá cumplir con los requisitos del programa, no envíe su formulario de registro.
6. También nos gustaría recordarle que se dará prioridad a los feligreses registrados ACTIVOS de la Parroquia de la Catedral de Cristo. Si queda espacio después del último día de registración que es el 10 de agosto, se considerarán a los que no feligreses en el orden recibido.
7. El equipo de confirmación comenzará a revisar los formularios de inscripción la semana del 11 de agosto y enviará un correo electrónico a los solicitantes informándoles si su inscripción ha sido aprobada o negada. Recuerde, el espacio es limitado y debemos darle prioridad a aquellos que son feligreses registrados y ACTIVOS.
8. ATENCIÓN: Su formulario de registro será NEGADO si NO proporcionó lo siguiente:
a. Acta de Nacimiento del estudiante.
b. Certificado de Bautismo del estudiante.
c. Certificado de Primera Comunión del alumno. (si es aplicable)

Hướng dẫn điền đơn trên mạng
1.Văn phòng sẽ có đơn trên mạng bắt đầu từ ngày 7 tháng 6 tới hết ngày 21 tháng 6.
(Nếu quý vị cần được giúp đỡ để điền đơn, xin đem theo máy tính di động hoặc máy điện tử cùng những giấy tờ cần thiết (theo yêu cầu phía dưới) tới Trung tâm Văn hóa cùa Giáo xứ, vào ngày 7 tháng 6 từ 9 giờ sáng tới 5 giờ chiều. Chúng tôi sẽ có nhân viên giúp quý vị.
2. Trước khi điền đơn, xin quý vị xem xét con em cùa quý vị đã hội đủ điều kiện sau:
a. Là học sinh trung học, đang học lớp 9 cho tới lớp 12
b. Con em của quý vị có thể đọc, viết và hiểu tiếng Anh lưu loát.
c. Con em của quý vị đã được lãnh nhận Bí Tích Rửa Tội theo Nghi thức của Giáo hội Công Giáo.
3. Những điều cần biết và giấy tờ cần thíết để điền đơn
a. Con em của quý vị sẽ có mặt và cùng điền đơn với quý vị
b. Có sẵn xố ký danh của gia đình trong Cộng đoàn Giáo xứ Nhà thờ Chánh tòa.
c. Có sẵn bản sao giấy Khai Sanh, chứng từ Rửa tội và chứng từ Rước Lễ Lần đầu trong máy tính của quý vị.
d. Nếu con em của quý vị cần sự giúp đỡ đặc biệt, xin có sẵn giấy tờ chứng nhận từ học khu ngày thường (IEP), trong máy tính của quý vị.
e. Dành riêng cho các em ghi danh lớp Thêm Sức năm 2: có sẵn thẻ ngân hàng hoặc thẻ tín dụng để thanh toán lệ phí là $350, sau khi đã nhấn vào nút “Gửi mẫu đăng ký”.
f. Dành riêng cho các em ghi danh lớp Thêm Sức năm 2: nếu con em của quý vị đã học lớp Thêm Sức năm 1 tại các giáo xứ khác và quý vị muốn con em mình đăng ký và học lớp Thêm Sức năm 2 tại Giáo xứ Chánh tòa Chúa Kitô, xin có sẵn thư chuyển nhượng trong máy tính cúa quý vi.
4. Mỗi học sinh cần điền một mẫu đơn riêng biệt
5. Xin xem xét kỹ những điều kiện của Chương trình Lớp Thêm Sức dành cho năm thứ 1 hoặc năm thứ 2. Nếu quý vị thấy chưa hội đủ điều kiện, xin hoãn lại và không điền đơn này.
6. Xin lưu ý: Chúng tôi sẽ ưu tiên dành chỗ cho gia đình có ghi danh trong Cộng đoàn Giáo xứ Chúa Kitô. Nếu còn chỗ trống sau ngày hết hạn ghi danh là ngày 21 tháng 6, chúng tôi sẽ xem xét đơn của những gia đình không ghi danh trong Cộng đoàn Giáo xứ.
7. Bắt đầu từ ngày 22 tháng 6, văn phòng Lớp Thêm sức sẽ xem xét từng hồ sơ gồm những giấy tờ đã gửi tới, và sẽ gửi điện thư thông báo đến gia đình cùa quý vị cho biết hoặc là đơn đã được chấp thuận hoặc là đã bị từ chối. f. Những học sinh nào muốn chuyển từ các giáo xứ khác, xin có sẵn thư chuyển nhượng được kỳ bởi cha xứ hoặc điều phối viên của chương trình Giáo lý Thêm sức của giáo xứ mà học sinh đã học lớp Thêm sức Năm 1 (nếu có)
